If you don't mind a really fucked up UX, try http://frank.fuhlbrueck.net/ itfoss/flatboat/ it has annotations.  Don't know about the "home library" but calibre should be able to export to anything if you set up the right export rules.

I've used MuPDF and Document Viewer (from sufficientlysecure.org (http://sufficientlysecure.org)) already.  I know there are solutions for ePub, but I'm specifically asking about PDF.

The permissions are ridiculous, but for example, Moon+ Reader seems to have the options I desire:

and from the photos, it has a nice UI.  Alas, it is far and away from FOSS.
